Policies regarding faculty leaves of absence and teaching relief for all Yale schools are located in the Faculty Handbook, section XVII. Note:ForproceduresintheSchoolsofMedicineandPublicHealth,pleaseconsulttheOfficeofFacultyAffairs,YaleSchoolofMedicine(http://medicine.yale.edu/facultyaffairs/index.aspx).  GeneralProcedures: Thefacultymembersubmitstherequestusingtheform,“RequestforFacultyLeaveorTeachingRelief”:          AcademicYear2022-23versions: FacultyofArtsandSciences2022-23 ProfessionalSchools2022-23       AcademicYear2021-22versions: FacultyofArtsandSciences2021-22 ProfessionalSchools2021-22 AllrequestsarethensubmittedtoDianeRodrigues([email protected]),OFAS,foraneligibilityreview.  OFASwillsendtoeithertheFASDean’sOfficeortheProvost’sOfficeforapproval. InFAS,thedeanssendapprovalsdirectlytothefacultymember. Intheprofessionalschools,approvalsaresenttotheschooldeantocommunicatetothefacultymember. Templateapprovallettersareprovideduponrequest. Uponapprovaloftheleaverequest,thedepartmentadministratorenterstheleaveinWorkday. Forassistancewiththisbusinessprocess,refertotheWorkdayguide,“LeaveofAbsence:Academic”.  Note: AllacademicleavesmustbeginoneitherJuly1orJanuary1andmustendoneitherJune30orDecember31. Exceptionsaremadeforfacultytakingunpaidleavesandneedtoreceivesummercompensation.  FordetailedproceduresoftheFASacademicleaveprocess,refertotheFASDean’sOfficesitefornon-tenuredladderfacultyleaves. Cancellations: Tocancelaleave,[email protected],aswellastheFASDean’sOffice([email protected])ifnotyetcommunicated.  OFASwillrescindtheleaveinWorkday.  Changes:  RequestsforchangesmustberesubmittedtoeithertheFASDean’sOfficeortheProvost’sOfficeforapproval. IftheleavehasalreadybeenenteredinWorkday,OFASwillmakethenecessarychanges. NOTE: CostingchangeswillneedtobeupdatedmanuallyinWorkdayifchangesaremadetoresearchleaves. Correctionstoleavedatesdonottriggeracostingtask. Additionalproceduresforrequestingcertainleavesortimeoff JuniorFacultyFellowships,MorseLeaves,ReappointmentLeaves,andAssociateProfessorLeaves-Thesearepaidresearchleavesfortenure-trackfaculty(AssistantProfessorsandAssociateProfessorsonterm). Theproceduresforapplyingfortheseleavesareestablishedandcommunicatedbyeachschool’sdean’soffice.  FASproceduresareonlineathttps://fas.yale.edu/book/faculty-leaves ProfessionalDevelopmentLeave(FASonly)-Thisleaveisofferedtoinstructional(non-ladderteaching)facultyinFAS. Adescriptionofeligibilityandtheproceduresforapplyingareonlineathttps://facultyadmin.yale.edu/resources/leaves-teaching-relief/fas-professional-development-leave-policy-non-ladder-faculty. Secondment-SubmitaletterormemoofapprovalfromtheProvost’sOffice. Short-termmedicaldisability-Submitaletterfromthefacultymember’sphysiciandocumentingthenatureanddurationofthedisabilityinordertodeterminetheappropriateaccommodationandarrangements. Thelettermustcontainthefollowingdetails: •  Letterheadfromthephysician’spractice •  Startandanticipatedenddates •  Jobfunctionsthatareaffectedbythemedicalconditionoritstreatment •  Confirmationthattheemployeeisunabletoperformessentialfunctionsofhisorherjob •  Frequencyoftimeoff–intermittentorcontinuous(ifintermittent,thenumberofhoursperdayand/ornumberofdaysperweek) •  Accommodationsneeded,ifany Foradditionalinformation,refertotheFacultyHandbook,sectionXXI.E. SpecialTriennialLeave-Requiresproofoffundingfromanexternalfellowship. Thefundingmustcoveraminimumofoneterm,therefore,alongwiththeleaverequestform,thefacultymembermustprovidedocumentationthatincludesthedatescoveredbythefunding. TeachingReliefforChildRearing-ThefacultymemberonlyneedstosubmittheRequestforFacultyLeaveandTeachingReliefandincludethedateofthebirthoradoption. Notesregardingunpaidleaves: Whileonunpaidleave,thesystemwillpreventthefacultymemberfromreceivinganycompensationfromYale,includingsummercompensation. Therefore,ifthefacultymemberneedstoreceivesummercompensation,[email protected]. Healthcoveragewillcontinuewithoutinterruption.ThepremiumswillbecoveredbyYaleandwillgointoarrears.Whenthefacultymember’scompensationresumesaftertheleave,theamountowedwillbedeductedfromthefacultymember’spayonemonthatatimeuntilfullyrepaid. Note: Forfacultymemberswhoplantobeoutoftheservicearea,theymaychoosetosuspendtheirhealthbenefitsduringthetimeaway. Ifso,theyshould contactEmployeeServicesforassistancewiththeprocess. Deferredretirement(YURAP)andthematchingcontributionswillceaseduringthistime. Foranyotherautomateddeductions,suchasparkingorloans,thefacultymemberwillneedtocontactthesourceofthedeductiontomakealternativearrangements.
