Bileisfluidmadebytheliverandreleasedintothesmallintestinetohelpdigestion.Changesinbileconcentrationandcompositioncausedbychangesindiet,hormones,medications,orrapidweightlossorweightgaincanresultinformationofsolidpiecesofbilecalledgallstones.IntheUnitedStates,morethan6millionmenand14millionwomenhavehadgallstonedisease,withhigherratesinNativeandHispanicAmericans.Asmallportionofpeoplewithgallstonesmayexperienceoccasionalabdominalpain,nausea,andvomiting,oftenaftermeals. Gallstonescansometimesmigrateoutofthegallbladder,blockthenormalflowofbile,andleadtoinflammationandinfectionofthegallbladder.Thisiscalledcholecystitisandcancausesharp,constantabdominalpain,fever,nausea,andvomiting.Thesesymptomswarrantavisittoaphysician,whocanarrangeforanultrasoundandmakeareferraltoasurgeonifappropriate. Gallstonesdonotneedtobetreatediftheyarenotcausingproblems.Patientswhohavecholecystitisorarebotheredbysymptomsofgallstonesaretreatedwithsurgicalremovalofthegallbladder,knownascholecystectomy.Laparoscopiccholecystectomy,alsoknownasminimallyinvasivecholecystectomy,isperformedthrough4smallincisionswithuseofacameratovisualizetheinsideoftheabdomenandlongtoolstoremovethegallbladder.Surgeryisdoneunderanesthesia,andpatientsareasleepthroughouttheentireprocedure. Mostpatientsgohomeonthedayoforthedayaftersurgery.Patientscanreturntoeatingtheirusualmealsathome.Mildtomoderatepainiscommonforafewdaysandcanbemanagedwithpainmedicationsasprescribed.Dressingsontheincisionscantypicallyberemovedthedayaftersurgery,atwhichtimepatientscanresumeregularshowers,butsomeincisionsmayrequirespecialcareasinstructedbythesurgeon.Afewpeoplemayexperiencediarrheaaftercholecystectomy,whichgoesawayafterafewweeksormonthsinmostpeople. Complicationsafterlaparoscopiccholecystectomyarerarebutcanbeseriouswhenstructuresnearthegallbladderaredamaged.Oneofthesestructuresisthecommonbileduct,whichisthemaintubethatdrainsbilefromtheliverandgallbladdertothesmallintestine.Damagetothecommonbileductorotherbiliaryductscancauseleakageorobstructionofbileflowandhassymptomsofabdominalpain,fever,andyellowingofskin,knownasjaundice.Thesesymptomscanalsobecausedbyagallstonethatisleftbehindinthebileduct.Bleedingfromabloodvesselnearthegallbladdercansometimesoccur.Mostbleedingstopsonitsown,butrarely,persistentbleedingrequiresintervention.Aftercholecystectomy,patientswhoexperienceworseningpain,fever,chills,nausea,vomiting,orjaundiceshouldseeadoctor.
