Thedevelopmentofthisguidelinewillfollowanexplicitprocessofevidencereviewandappraisal.Treatmentapproachesareaddressedineachchapterandguidelinerecommendationsarebasedonsystematicreviewsofrelevanttrials.Practicalcommentsorstatementswhichserveaseducationalpurposesareungraded,butincludedasimportantinformationforthereadership.AppraisalofthequalityoftheevidenceandthestrengthofrecommendationsfollowedtheGRADEapproach.Ongoingareasofcontroversies,limitationsoftheevidence,andinternationalrelevancewillbediscussedandadditionalsuggestionswillbeprovidedforfutureresearch.
