Overproductionofaldosteronecausesthebodytoretainmoresodiumandlosepotassium,whichleadstoelevatedbloodpressure.Patientswiththisconditionareatincreasedriskofheartdiseaseandstroke. Symptoms Themainsymptomexperiencedbypatientswithprimaryaldosteronismismoderatetoseverehighbloodpressure,whichisoftenresistanttodrugtreatmentsandaccompaniedbylowpotassiumlevels. Othersymptomsinclude: Fatigue Headaches Muscleweakness Numbness Sporadictemporaryparalysis CausesandRiskFactors Manycasesofprimaryaldosteronismarecausedbyabenigntumorinanadrenalgland;somearecausedbyoveractivityofbothadrenalglands,aconditionknownasbilateraladrenalhyperplasia. Inrarecases,theconditionmaybecausedbycanceroustumorsintheouterlayerofanadrenalgland,orbyageneticconditionknownasglucocorticoid-remediablealdosteronism. Theconditionismostcommoninpeoplebetweentheagesof30and50. Diagnosis Whendiagnosingprimaryaldosteronism,thephysicianoftenwillfirstorderabloodtesttodeterminethepatient’saldosteroneandreninlevels.Reninisanenzymereleasedbythekidneysthathelpsregulatebloodpressure.Ifelevatedamountsofaldosteroneandminimalamountsofreninarefound,thiscouldbeasignofprimaryaldosteronism,andothertestsmaybeorderedtoconfirmthediagnosis. Thesetestsmayincludesaltloading,whichinvolvestheintakeofhighlevelsofsodiumfollowedbybloodandurinetestingtodeterminehowthebodyishandlingthesaltandrespondingwithaldosteroneproduction. AdditionaltestingcanincludeComputedTomography(CT)orAdrenalVeinSampling(AVS),inwhichanexperiencedradiologist,suchastheprofessionalsatCedars-Sinai’sHypertensionCenter,usesacathetertotakebloodsamplesfrombothadrenalglandstomeasurealdosteroneproduction.  Treatment Whenprimaryaldosteronismiscausedbyabenigntumorontheadrenalgland,theconditionmaybetreatedeithersurgicallyorwithmedication. Ifsurgeryisthebestoption,anadrenalectomycanbeperformedtoremoveboththetumorandtheadrenalglandtowhichitisattached.Thisoftenpermanentlyresolvestheprimaryaldosteronismandthephysicianwillfollowthepatient’sprogressclosely,adjustingoreliminatingthepatient’smedicationsasneeded. Whensurgeryisnotanoption,aldosterone-blockingmedicationscanbeusedtomanagethecondition.However,thesedonotcuretheconditionandmustbetakenregularlytomanagethesymptoms.
