現今,阿茲海默症已佔所有失智症一半以上,也成為老年人健康問題的第三名。

在美國,阿茲海默症是死亡原因的第六名,估計2012年間照顧者可能為照顧各種失智症患者花費200億美元。

迄今,阿茲海默症的機轉亦未完全了解。

「類澱粉階梯理論」(amyloidcascadehypothesis)敘述了阿茲海默症形成之步驟。

然而目前美國食品管理局核可治療阿茲海默症藥物有二種,一為乙醯膽鹼水解醑制劑,另一為NMDA受體拮抗劑。

雖然研究顯示此兩類藥物均比安慰劑但更可維持認知功能,但是它們僅是改善疾病過程,並有許多副作用。

本研究基於「類澱粉階梯理論」(amyloidcascadehypothesis)而設計,利用乙型類澱粉胜(b-amyloidpeptide;Ab)經由立體定位與微小幫浦注入大鼠腦室誘發行為及學習障礙,藉以了解益氣聰明湯對乙型類澱粉胜誘發行為及學習障礙改善作用。

而研究結果發現在分組治療與對照組服用益氣聰明湯的老鼠學習及行為比對照組好很多,而且鼠腦切片分析不只發現乙型類澱粉斑塊也減少,乙醯膽鹼分泌與NMDA受體亦有增加之情形,所以顯示益氣聰明湯可減輕乙型類澱粉誘發的學習障礙。

AloisAlzheimerdescribedthetypicalsymptomsandsignsofdementia,andfirstnotedthecharacteristicpathologicpictureinthedementiabrainonehundredyearsago.Sofar,Alzheimer’sdiseaseisresponsibleforabove50%ofallfindsofdementia,isalsothethirdgreatedheathproblemintheelders.InAmerican,Alzheimer’sdiseaseisthesixthleadingcauseofdeath,aswellpaymeatsformedicare,longtermcareforAlzheimer’sdiseaseandotherdementiaareanticipatedmorethan200billionUSdollarsin2012.TheprecisemechanismbehindAlzheimer’sdiseaseisnotyetdisclosedcompletely,「amyloidcascadehypothesis」isthemostpredominantmodelforunderstandingthepathophysiologyofAlzheimer’sdisease.However,therearetwotypeofmedicineswereprovedbyUSFDAforthetreatmentofAlzheimer’sdisease;acetylcholinesteraseinhibitorsandN-methyl-D-aspartatereceptorantagonist.Thoughbothofthemcanmaintainhigherlevelofcognitivefunctioncomparedwithplacebo,theyareonlydisease-modifyingwithmultipleadverseeffects.Ourexperimentwasdesignedbasedon“amyloidcascadehypothesis”,thatiswidelyusedforexplanationofAlzheimer’sdiseasepathogenesis,whoseanimalmodelwithAlzheimer’sdisease-likephenotypewasdevelopedbyinfusionA(1-40)inratsbrains.Therearetwoprotocolsinourstudy,oneispreventive,theotheristherapeutic;whichourmedicationisYi-Chi-Tsung-Ming-Tang(YCTMT),adecoctioniscomposedofAstragaliradix,Ginsergradix,Pueratiaeradix,Paeoniaelactiforae,Phelloadendriconex,Viticisfructus,CimicifugaeuralensisandGlycyrrhizauralenss.OurresultsdemonstratedtheameliorativeeffectofYCTMTonA(1-40)inducedlearningimpairment.OraladministrationofYCYMTonratsappearstobeabletoprotectA-impairedmemoryretention,spatiallearningandmemory,aswellpromoteexpressionofacetylcholineandNMDARs.Therefore,thetherapeuticprotentialofYCTMYinAlzheimer’sdiseasemeritsfurtherinvestigation.
